Striking and Fielding Event at AKS, Lytham

Dear Parents/Carers, 

We have entered a team of 10 Year 3/4 children to take part in a Striking and Fielding Event at AKS, Lytham alongside other local primary schools.  

The event will take part on Monday 20th May between 1.15pm - 3.15pm and we will leave school after an early lunch.   Children will be transported to the venue via mini bus or staff cars (to be confirmed) and returned to school if required (please indicate below).  Please can children come to school in full PE kit on this day. 

We hope that you agree this will be a fantastic opportunity for the children.  We require parental consent for your child to attend this event, please return this form asap or by Friday 3rd May 2024.  

If your child travels using a child seat/booster can you please leave this with Mrs Hill on the gate at morning drop off.  

If you do not give consent for your child to attend this event please complete the form accordingly and arrange to discuss further with Mrs Cameron.  We will need to establish the reasons why and agree where your child will be working within school for the duration of the trip.

Declaration by Parent/Carer:

I agree to my son/daughter/ward taking part in the above stated visit/activity and having read the information letter, agree to his/her participation in any or all of the activities described.  I acknowledge the need for good conduct and responsible behaviour on his/her part and that the school/service reserves the right to prevent my son/daughter/ward taking part in the visit/activity in the case of poor behaviour.  Further, I understand that there would be no entitlement to a refund of monies paid.  I agree that I will update the school/service with any medical information or changes to emergency contact details.
